The admission department at Friends Hospital is operated 24/7 by specially trained staff who provide confidential assessments and refer people to the appropriate level of care. We are also available to referring professionals such as outpatient clinicians, social service agencies and other hospitals seeking treatment for a patient.

To make the admission process go as smoothly as possible, please download and complete our authorization completion instructions and authorization to obtain or release protected health information form.

Please bring the following important documents at the time of admission:
